Lufia II: Rise of the Sinistrals

The boss "Egg Dragon" has a programing error. The boss has the highest amount of HP possible in the game (65535 HP), however, due to a lack of an HP cap, if you heal the boss it will reset his HP to a low number. After healing him, it's possible to defeat him in a single hit.

Gandar's starting Hero Value is a very low -106, but this is irrelevant, since he cannot be transferred to Asgard; despite the fact that Odin expressly asks for him, his personality is simply not fitting for an Einherjar (it is likely that Odin only asked Lenneth to recruit him so that Hel would not be able to use his powers).

Devil Survivor Overclocked

According to director Shinjirō Takata, the new voice acting for the 3DS release of Shin Megami Tensei: Devil Survivor, subtitled Overclocked, included around 20,000 voice lines, of which about 15,000 covered the game's original dialogue and 5,000 covered the new "Day 8" segments. At the time, this was the most voiced dialogue in any game released by Atlus.

During the first fight with Leon, it is possible to defeat him either through cheating or by grinding to at least level 12 beforehand. Beating him will trigger a short cutscene showing a secret ending to the game where Spoiler:Rutee briefly lambasts Leon and then forcibly enlists an upset Stahn to join her and Mary in becoming an infamous trio of globe-trotting Lens hunters, before the game abruptly returns to the title screen. This ending is not featured in the game's PlayStation 2 remake, as Leon's fight is redesigned to be impossible to defeat.

The Legend of Zelda: Phantom Hourglass

Phantom Hourglass contains a an island called "Dee Ess Island" - an island in the shape of the DS Lite when shown from the sea and an original DS when shown from the island itself. The infamous American Mega Man 2 box art, where Mega Man wields a pistol instead of his signature Mega Buster, was not an attempt to make Mega Man appear more "edgy", but was actually born from a misconception where Capcom's art director believed Mega Man was using a gun too small to be seen in sprite form rather than transforming his arm. Artist Marc Ericksen would say to Nintendo Age in 2012:

"What happened was I had to go down to Capcom and do a game they just got in from Japan. They had a beta version; they were in a hurry to get this thing out. So I went down there. The gentlemen was the art director was there with the beta guy and they were waiting for me and I came with my pad and they started running the game. What we saw was this little pixelated figure of the famous Mega Man running around on the screen shooting. Bang, bang, bang, he's shooting obviously like this with his arm [Marc points his arm forward]. So I said to the art director, 'what is he shooting? What is he shooting with?' The art director said, 'he must have a pistol because I don't see that he's got a rifle so he must have a pistol.'

'So... a pistol? You want me to do a pistol?' And he said, 'yeah, let's put a pistol in there.' So I did what I was told and I put the pistol in there. Add to the fact that they only had, like, a day and a half for me to do the painting and what you wound up with was not the greatest result but certainly a result that was not my fault. It was one of those things. Here's my opportunity: I'm saying to everybody now that was not my fault! Talk to Capcom about the pistol."

Real Bout: Fatal Fury

SNK created new assets for the loading screens on the Neo-Geo CD version. However, the art style used for said loading screens was considered "too eccentric" by many fans, to the point that they sent letters directly to SNK demanding that they shouldn't use the style anymore. Because of this, the art style used in those loading screens was never used again in future SNK games.
