For what it's worth, reading through a number of reviews as I've been updating the OP, I don't think it's like this either.
Apparently these are the changes in New Game+, and I will mention these stack so there's more changes in four different playthroughs (explain more in detail afterward):
-New areas in many previous locations
-New bosses sprinkled throughout (one review mentioned they were half-way through their second playthrough & had encountered two new bosses along the way).
-New documents about
-Changed and whole new cutscenes (apparently if you try skipping a cutscene that's changed, the game will warn you the cutscene has new/changed stuff in it and ask if you really want to skip it)
-New side quests open up
-New items/weapons/skills, along with previous skills carrying over from past playthroughs
-New skips, you get new items which skip certain sequence of the original game and some lead to completely new areas/bosses
But this isn't split between "New Game" and "New Game+", more accurately the game has four "routes" or "playthroughs" which each time adds more & more changes to the experience. Roughly, the structure of the game goes like this:
-Playthrough 1: One ending, linear story
-Playthrough 2 & 3: Three new endings open up, the story now branches out and there's various remixed stuff, new notes, new cutscenes, new bosses, new side quests, etc. Two new "main" endings you can get in either order, and one additional "joke" ending separate of those you can get while trying to get another ending with a well planned save file (since you just need to finish its unique side quest to get the ending, then can reload your save to get back to getting the "main ending" you're on after finishing the joke ending side quest & getting it, can do any time on New Game+). Once you get one of the two new "main endings", the third playthrough adds even more changes to the game than the 2nd playthrough, including more skips, new areas, notes, bosses, cutscenes, story, etc, etc.
-Playthrough 4: After getting all three "main endings" (Joke ending isn't counted towards this), you unlock a final ending and a "final" remixed version of the game with more new content added throughout the game leading to the final ending.
Is roughly how it seems to work. It's not just the ending and end game that changes, but the road there as well.
Cosa c'entro io? Silent Hill Automata and Replicant with Knuckles

Wow, sarebbe figo.
